Worker entangled in unguarded machine

David Humphries was drilling a hole in a stock bar at Tyre Renewals when his trousers became entangled in the rotating feed rod of the lathe. Although minor injuries were sustained, Mr Humphries died of a heart attack shortly after the incident.
